Registration & Testing Instructions

DOT Drug & Alcohol Testing / CDL Physicals

To begin your testing process, please proceed to complete the Donor Information / Registration section on the next screen.

The zip code you provide will be used to identify the closest authorized testing center for your convenience. Once your registration is complete, you will receive a Donor Pass / Registration Form via email. This document will include:

  • Testing center location and address

  • Hours of operation

  • Step-by-step instructions for your visit

Please bring a copy of this form with you—either printed or accessible on your smartphone—to present at the testing facility.

In most cases, no appointment is required, allowing for a streamlined and efficient testing experience.

DOT Consortium Enrollment

If you have selected enrollment in the DOT Consortium, you will receive a Certificate of Compliance via email immediately following registration. This certificate serves as official documentation confirming your participation in a compliant random testing program.

If your enrollment also includes a DOT drug test, a separate test registration form will be issued. This form will provide your assigned testing location based on the zip code submitted, along with all necessary instructions for completion.

Important Notice

Payment is required at the time of registration in order to process your request and release all testing documentation and compliance materials.
